Like for example, there's never a proper vantage point where you can survey the enemy and make a move, things are kinda just thrown at you as you progress which implies that the game wants to be a shoot em all. It always felt like it was designed for a different playstyle (I chose stealth). My main gripe with the game is the level design. And ofcourse it has the whole psychological aspect going on as well. The librarian part is a nightmare, and the other mutants are frustratingly hard to kill. The guns are wonky but realistically so, it fits the mood. NPCs aren't shit, abd actually add something to flesh out the world. And it isn't afraid to make you feel uncomfortable. Be it the parts on the surface or in the tunnels, the whole thing felt more than a presentation in a game. It does a great job of pulling you right in.
